Tyler Glenn Rivera-Jacobs, 15, of 3325 Five Bridge Road, passed away at his home, Saturday, May 21, 2016.

A funeral service will be held at 3 P.M., Tuesday, May 24, 2016 at Holly Grove Holiness Church with Pastor Lynn Blackburn, Rev. Dr. Randy Simmons and Reverend Ricky Jacobs officiating. 

Tyler, born in 2001 in Sampson County, was the son of Crystal Gail Jacobs and the late

Ricardo Garica Rivera.  He was a rising 9th grader at Clinton High School and worked with Wayne Pearson Farms.

In addition to his mother, he is survived by his sister, Savannah Lee Rivera Jacobs of the home; maternal grandmother, Linda Faircloth and step-grandfather Junior Faircloth; maternal grandfather, Glendale  Jacobs and step-grandmother, Debbie Jacobs; uncles, Danny Rivera and Dr. Mike Ammons; aunts, Dana Ammons and Renee Jacobs; nieces, Idalis Jacobs and  Ava Ammons; step-aunts, Kimberly Simmons and Brittany Drees; and special loved ones, Jason Evans and son Ayden.

The family will receive friends, from 6 P.M. – 8 P.M., Monday, May 23, 2016 at Holly Grove Holiness Church at  1519 Indiantown Road, Clinton, NC  28328.
